A clearance is not a formality. It is the only point in the job where the result is measured instead of described, and it is far cheaper to find a missed wet cavity while the plastic is still up than after the drywall, trim and flooring have gone back in.

Framing, sheathing, sub-floor and any adjoining material are read and compared against dry material elsewhere in the building. A space that passes on air but reads wet in the studs is not finished.
Where sampling is part of the scope, air samples inside the containment are compared with an outdoor control, and surface samples are taken from cleaned material. The lab report goes to you, not just to the contractor.

Where the work area was, what was removed, what the readings were, what was sampled and what the lab returned. It is the document a buyer, a landlord or an insurer will ask you for later, so keep it with the property records.
The area is re-cleaned or opened further and tested again. That is the normal use of a clearance, not a failure of the job, and the scope should state up front who pays for a re-test and under what circumstances.
What a clearance costs follows the number of separate work areas, whether laboratory samples are included or the check is visual and moisture only, and how far the assessor has to travel between rooms in a large or commercial building. Multi-unit property usually needs a clearance per unit rather than one for the building.
Timing matters as much as price. The test belongs after the cleaning and drying are finished, with the containment and the air filtration still running, and before a single sheet of drywall goes back. Booked the other way round, it measures a room you have already rebuilt.
If nothing has been measured yet, begin with a mold inspection instead. It is not required on every household job, but it is the only proof the work reached the end of the wet material. If the property is rented, being sold, insured against the loss or occupied by someone with asthma or allergies, get it in writing.

Not usually. The result comes back after the samples are processed, and the rebuild follows that. Anything you took out of the room should stay out until the report is in hand, because putting stored contents back into a space that has not cleared undoes the point of the test.
